Prospective/future record access

You may have had correspondence from your GP practice, or read in the media about Prospective/future record access. This refers to a plan to provide patients with online accounts, who do not already have access, with digital access to their medical records from the end of November 2022. The access is to all areas of your medical record, including documents and test results (once they have been assessed and filed by your practice).

This applies to Patient Access, as well as the NHS App and other online services providers.

The reason for the change is that evidence has demonstrated that better access to health information enables patients to become partners in managing their health and long-term conditions.

This is an NHS England programme supported by NHS Digital.

The change, when enabled, will not affect:

Those under the age of 16
Proxy accounts

Some information may be restricted from a minority of patients due to safeguarding concerns. Your GP practice manages your online access to your medical record, please contact them if:

You think that there is any missing information
You had access to your medical records before the change and can now see less information
You have any other concerns

COVID-19 Vaccination Card

Please note, this new feature is only available to Patient Access users in England.

The COVID-19 Vaccination Card makes it simple for you to view your COVID-19 vaccination record within the Patient Access website and app.

The COVID-19 Vaccination Card is visible in the Immunisations section of your Medical Record.

The card is only visible if your practice have enabled access to Medication and Immunisations in your detailed care record. It will be available if you have received either the first or second doses of the vaccine and will include the name of the vaccine and date administered.

For those patients who do not have their detailed care record and immunisations enabled, the new feature will not be automatically visible in the Patient Access website and app. If you do not have this access, you can speak to your practice.

Share your medical record

Certain services, such as viewing your medical record, nominating a pharmacy, changing contact details and messaging, are only available to practices in England and certain parts of Northern Ireland at this time. Your practice decides which of the available services they make available to their patients.

Sharing medical records using Patient Access is an easy way for you to quickly and securely give temporary access to your medical record. The shared record will be available for 24 hours via a unique link and secured using an access code. You can select the areas of your medical record you would like to share and have the option to stop sharing access at any point.

Sharing and/or exporting your medical record is only available if your organisation has enabled access to your medical record.

Benefits of sharing

Patients can temporarily share their record with family and friends or healthcare professionals giving them the opportunity to:

Keep family and friends informed.
Share medication and allergy histories with a nursing home.
Get the most efficient care in an emergency.

How to share your record

If you would like to share your record, follow the steps below. You can also view our video tutorial on how to share your record here.

Sign in to Patient Access.
Select Medical Record on the dashboard to expand the selection.
Select Share Record.
Select the areas of your medical record you wish to share.
Select I accept the terms and conditions, and then select Share.
You have successfully shared your medical record.

You can also share your record by selecting the link in all areas of the medical record except documents. Documents cannot be shared.

You will be provided with the following information:

The share link which you can either copy (then paste into an email) or you can select Email link to generate an email automatically.
The access code, which needs to be entered by the recipient of the email (or QR code).
Note: After the shared record has been viewed for 10 minutes, the code will need to be entered again.
The QR code which can be scanned with a smartphone or tablet.

Shared record history

You can share the link with as many people as you like, the shared record history shows the date and time it was created, the areas shared, the access code and the number of times a link has been viewed.

Stop sharing

You can stop sharing at any time. You can stop sharing one instance of your shared record by selecting Stop sharing on the individual shared record. If you select Stop sharing all this will stop access to all the shared records you currently have active.

Stop sharing will be replaced with Expired when either the 24 hours has elapsed, or you have chosen to stop sharing the record.

Export your medical record

Exporting your medical record using Patient Access is an easy way for you to share your record with family and friends or healthcare professionals. You can select the specific areas of your medical record you would like to export, e.g. medications, and export the information into document form.

Sharing and/or exporting your medical record is only available if your organisation has enabled access to your medical record.

How to export your record

Sign in to Patient Access.
Select Medical Record on the dashboard to expand the selection.
Select Export Record.
Select the areas of your medical record you wish to export.
Select I accept the terms and conditions.
Select the format required, for example, PDF.
A warning appears indicating other data that will be exported.
Select Export.
The record is exported and will open automatically at the bottom of your screen.

View your medical record

If you have medical record enabled, there will be a medical record section on the Patient Access dashboard once you've signed in. To view your medical record, simply select Medical Record from the dashboard to expand the selection, then select the area you would like to view.

Depending on what your practice have enabled you may be able to see your:

Problems
Medications
Test results
Documents
Consultations
Immunisations
Allergies

What you see and do on Patient Access is controlled by your practice and they decide which areas of your medical record you can see. If you'd like to see more of your medical record, speak to your practice about this.

COVID-19 Vaccination Card

If you are a patient in England, you will be able to access your COVID-19 vaccination information from the Immunisation section of your Medical Record. Read more about the COVID-19 Vaccination Card here.

For any information to be displayed, you must have had at least one dose of the vaccine. COVID-19 vaccination information will only be displayed if you have access to your medication and immunisations.

Viewing options

Filter

Select Show: All records in any area of the medical record, other than Problems and Medications, to just display data between particular dates or a particular range.

Select from:

To and From specific dates.
This Month.
Last Month.
This Year.
Last Year.

Select Filter: Show all, select the required dates or option, then Apply filter. To remove the filter and show all data, simply click x at the end of the date range displayed.

Select Share in any area of the medical record, other than documents, to temporarily share your record with family and friends or healthcare professionals. Documents cannot be shared.

You can also access the option to share your record by selecting Medical Record on the dashboard to expand the selection, then Share Record.

Grid or list view

In any area of the medical record, other than Test Results, you can select from either a grid view (default setting) or a list view. This simply changes the way the data in your medical record is displayed online and can be changed at any time.
